Glaucoma is a chronic disease that can lead to blindness if it is not detected early on. When glaucoma happens or starts off, it doesn’t have any symptoms in its typical presentation so, this is why it is important for everybody over the age of 40 to see an optician regularly. That’s one way where regular eye examination can detect if the pressure within the eye is high enough. If the pressure is high within the eye and it is not treated or lowered, then the high pressure can cause damage to the optic nerve. Consequently, if the optic nerve is affected by the high pressure, it stops working, and it leads to blindness.
Glaucoma can have different atypical forms. The most common type of glaucoma does not have any symptoms; it is only by checking that we can identify if the disease is starting off. Other types of glaucoma are less common; however, some types can be presented acutely and with symptoms like pain and headache and red-eye, where the pressure goes high all of a sudden, and this type of glaucoma is called acute angle-closure glaucoma. So, when there is angle-closure glaucoma, it is a medical emergency because if the pressure is not lowered within an hour or so, it can lead to permanent damage to the optic nerve and blindness. Open-angle glaucoma is the most common type of glaucoma, and like angle-closure glaucoma, it has no earlier symptoms.
The optician determines the ways through which glaucoma can be detected. However, there are common ways to detect this eye disease such as checking the intraocular pressure, examining the visual fields, the extent of damage or whether the fields are intact, an examination of the back of the eye to look at the optic discs which is the front part of the optic nerve and if there is any damage typical of glaucoma which is another way to identify if the disease is starting off.
If there is a narrow anterior chamber angle that is potentially at risk of angle-closure, then a preventive laser treatment called laser iridotomy can create a small opening within the iris and acts potentially like a safety valve. If the angle does not go into closure and the pressure is rising, it can only rise so much and then the water will flow from the posterior to the anterior chamber preventing a block and the pressure will never go that high to lead to full-blown angle closure and blindness. So if you are diagnosed at risk of angle-closure, we recommend you to have a laser iridotomy treatment to prevent this type of glaucoma. Other types of glaucoma are congenital glaucoma, which can be present at birth and glaucoma condition, which can intricate with inflammation, steroid treatment for a long time, trauma to the eye, and various other conditions.
So many factors cause glaucoma; some of them are unknown. The biggest factor for glaucoma is the pressure in the eye, which is the intraocular pressure. This is the one single factor that you can control and treat, and that’s how you manage glaucoma by keeping the pressure down. Another cause attributed to glaucoma is a genetic component to glaucoma in that if you have a family history of glaucoma, you are more likely to get this disease and certain people with conditions within the eye or generally such as diabetic or short-sighted people are more likely to get glaucoma.
By and large, there are three different glaucoma treatment options:
- One is the eye drops, and the eye drops work very well, but they rely on the patient having to put them in every day. Eye drops can give you local and systemic side effects, so do watch out for these side effects, and they also can stop working from time to time. Sometimes you need to use a combination of eye drops to get the pressure down because all of these medications work in slightly different ways.
- The second is lasers. The lasers have been around quite for a while, but nowadays, the new generations of lasers are very effective and very safe, painless, and quick. Some of the patients will be very suitable for laser treatment to lower the pressure. Some of these lasers work by removing the cells that make the fluid in the eye and some others work by increasing the drainage system within the eye to allow the fluid to drain better and keep your pressure down. These methods are nowadays, outpatient procedures with no downtime for the patients.
- The third is surgery. Surgery is very effective, but it is invasive. The standard trabeculectomy operation works very well and is still one of the go-to procedure when you really need to get the pressure down and save someone’s sight. There are also newer techniques which work just as well nowadays and are less invasive and quicker and obviously kinder to the eye as well. For example, the new microtube XEN procedures can do the same operation in under 5 minutes with no significant cuts or stitches to the eye. There are lots of new techniques on the market now, but the doctors have to evaluate them depending on what suitable for each individual person.
You might have heard of glaucoma surgery, but what’s exactly glaucoma surgery? In this procedure, your surgeon will perform glaucoma surgery to fix drainage problems in your eye due to high intraocular pressure. Glaucoma surgery options include laser and non- laser procedures, and they took approximately 10 to 60 minutes.
- Laser surgery: if you’re having laser surgery, you’ll be seated facing the laser machine, and eye drops will be administrated to numb your eye. Then, a small device will be placed in your eye to hold your eyelids open.
- Laser Trabeculoplasty glaucoma surgery: Trabeculoplasty is a laser procedure that increases fluid drainage from the trabecular meshwork. During laser trabeculoplasty, your surgeon will use a special contact lens held over the front of your eye to focus the beam of the laser onto the trabecular meshwork. This will allow more fluid to leave your eye, thus, lowering the intraocular pressure.
- Peripheral Iridotomy glaucoma surgery: Peripheral Iridotomy is a laser surgery that opens a narrow or closed drainage angle between the closed cornea and the iris. This closed-angle interferes with the drainage of the fluid. In this procedure, your doctor will use a laser to make a small hole in your iris so as to allow fluid flow more freely within your eye.
After glaucoma procedure, you’ll be monitored for one or two hours before being released. Also, you may be given an eye patch or bandage to wear temporarily. What’s more, you will be prescribed eye drops and instructed on their usage. Your eye doctor advises you to keep your Id drive for a time and avoid swimming or showering and to refrain from driving, heavy lifting, or straining temporarily.
